Alternative Living Arrangements
       On Site Visits

R&T provides information on retirement communities, nursing homes and other living alternatives.  R&T locates facilities in close proximity to the older adult's family and community, when possible.  The goal is to provide accommodations that match personality, interest, finances and the medical care needed.

Discharge Planning

R&T will coordinate the transition from hospital to home, meet with social workers, nurses, and doctors to determine the client's needs prior to discharge.  R&T will make arrangements for all services needed.

Monitoring & Telephone Reassurance

R&T monitors clients' health, social, emotional and mental status.  Monitoring is also provided to assure that the services ordered are being delivered and the quality of care is maintained.
Services are provided according to the need of the older adult and family, and on an agreed upon schedule.

Information and Referral

R&T provides assistance in locating community and out-of-town resources.
This service provides information and contacts for caregivers who are unable to do the leg work and research.
